The UPV Health Services Unit Miagao will start accommodating Miagao-based continuing Students (Graduate and Undergraduate) for the Annual Medical and Dental Examination for Academic Year AY 2026-2027.
Date Schedule: 23 March 2026 - 15 June 2026
Time: 8:00am - 12:00nn; 1:00pm - 5:00pm
Requirements:
1. Recent CBC and X-Ray results (completed not earlier than 2 weeks prior to the confirmed schedule for physical examination)
2. Completely filled-out Periodic Health Examination Form: HSU-Periodic Health Examination Form 2026
3. University ID / Form 5
UPV-HSU will use an online appointment platform for scheduling. Clients may access the reservation system at ereservation.onrender.com or by scanning QR code provided. For physical examinations, students may book appointments using their official UP email address.
