Academic Document Request

To request academic documents, please use the Online Academic Documents Request Tracking System (OADRTS) at https://crs.upv.edu.ph/documentrequest/. After submitting your request, note down your tracking code to monitor the progress/status of your request. Kindly wait for confirmation/final billing before proceeding with the payment process

Yes, we can issue a certified true copy of your diploma, provided we have a file copy in our records. However, for a certified true copy of the Official Transcript of Records (OTR), you must request first for the original copy of your OTR before we can process the certified true copy.

The processing time varies depending on the type and number of document/s requested and the volume of requests received by the office. Generally, requests for Official Transcript of Records (OTR) and other academic documents may take around 7–15 working days. Please see OUR's Citizens Charter for details. For urgent requests, you may call our office at (033) 3158556.

After submitting your request, wait for the confirmation/final billing before proceeding with the payment process. You can pay through the payment channels specified on the Online Academic Documents Request Tracking System (OADRTS) https://crs.upv.edu.ph/documentrequest/. If your documents need to be mailed, make sure to select the "to be mailed" option so that mailing fees are included in your total bill.
After submitting your request through the Online Academic Documents Request Tracking System (OADRTS) https://crs.upv.edu.ph/documentrequest/, you will receive a tracking code. Kindly track your request using the tracking code provided by revisiting the OADRTS and clicking "Track Request."
You can do this via Online Academic Documents Request Tracking System (OADRTS) at https://crs.upv.edu.ph/documentrequest/. Please indicate the name, complete mailing address, contact number and email address of the institution. After submitting your request, note down your tracking code to monitor the progress/status of your request. Kindly wait for confirmation/final billing before proceeding with the payment process. Once we received the Official Receipt (OR) of your payment, we will process your Official Transcript of Records (OTR) and other requested documents and send them to the specified institution.

Please be informed that the original diploma/honorable dismissal/certificate of transfer credentials is issued only once. We can issue a certified true copy of your document/s provided we have a file copy in our records. You may also request for the English version of your diploma.

Admissions

You are eligible to apply for first-year admission to UP through the UPCAT for SY 2025-2026 if:
  • You are a senior high school student expecting to graduate in 2025 in time for the opening of SY 2025-2026, OR you are a senior high school graduate from 2024 or earlier provided you have not taken any college courses before the opening of SY 2025-2026; AND
  • You have final grades in Grades 8 to 11 by end of SY 2023-2024; AND
  • You have not completed your application in UPCA 2021, UPCA 2022, UPCA 2023, UPCAT 2024 nor taken the UPCAT before.

A foreign OR Filipino applicant who has graduated or is graduating from a secondary school/home school abroad (i.e., international applicant) and has not enrolled in college may apply by automatic admission. Filipino students from Philippine high schools in the Middle East may also apply. For details, please visit the General Information Bulletin on First Year Admissions.

Students from other universities (T2) may apply for transfer to a UP campus after earning 33 academic units in another school with a weighted average of 2.00 or better on the UP marking system. The checklist of requirements and the procedure on how to apply are listed in the UPV OUR Citizen's Charter

REQUIRED ENTRANCE CREDENTIALS FOR UPCAT SUCCESSFUL APPLICANTS
A. First Batch of Requirements
  1. Two recent, identical 2x2 colored pictures with nametag and signature in front (a digital copy will not be accepted).
  2. Original High School Card (Form 138) duly signed by the Principal and bearing the remarks: "Graduated" and/or "Eligible for Admission to College," with date of graduation and Learner Reference Number (LRN).
  3. Original Student Permanent Record (Form 137) from Grade 7 to 12 with remarks "Graduated and Eligible for Admission to College" and "Copy for UP Visayas"(An official request can be downloaded here: Request for Form 137.
  4. Original Certificate of Live Birth issued by PSA.

B. Additional Requirements for submission to the OUR
  1. Medical Certificate issued by the UPV Health Services Unit
  2. Completely filled-out Student Directory (Update Personal Information in your CRSIS account. The instructions can be accessed here: Student Directory Confirmation. Please ensure that the details in the Student Directory will be the same information reflected in your PSA Birth Certificate)
  3. Certificate of Completion of Bridging Program, if applicable (An email will be sent to you if you are required to attend the bridging program in English, Math, or both based on your UPCAT result.)
  4. Special Power of Attorney (SPA) designating a Legal Guardian for first-year students whose parents are living outside Iloilo province and cannot respond immediately in case of emergency. Standard Format is downloadable at the UPV CRSIS website: Special Power of Attorney
The Admission Slip for enrollment will be forwarded to your respective colleges upon submission of complete ORIGINAL entrance credentials and additional requirements. Students with incomplete entrance credentials and additional requirements can enroll on PROVISIONAL STATUS, but submission of A-2, A-4, B-1, B-2 is a must. The submission of B-3 and B-4 requirements is also necessary, if applicable.
The decision to accept cross registrants is subject to the availability of slots in the courses offered this term and upon the approval of the offering college/school. In accordance with the Universal Access to Quality Tertiary Education Act of 2017, or R.A. 10931, all eligible students will be granted free tuition and miscellaneous fees when they enroll in the University of the Philippines. Students who fail to complete their bachelor's degree or comparable undergraduate degree within a year after the period prescribed in the program are no longer eligible to enjoy the privilege of free tuition and other fees. The "prescribed period" refers to the normal length of time to complete the requirements for graduation, e.g., 4 years, 4.5 years, or 5 years. Counting is done from the start of the student’s enrollment in UP.

Students who hold a dual citizenship (e.g., Filipino-American, Filipino-Chinese) and choose to be classified as Filipino during their initial enrollment will be considered Filipino until graduation. They can also avail of the free tuition. Prior to admission, these students must submit their identification certificates (ICs) issued by the Bureau of Immigration or their Philippine passports with either a report of birth or a reacquisition of citizenship.

UP Visayas is accepting applicants for non- degree/second-degree and transfer students only before the First Semester of the Academic Year.

No. Per University rule, students cannot shift or transfer if they have not earned the required number of academic units.

Shiftees are students who have opted to change either their degree program within the College (S1) or from one college to another within their current CU (S2). For UPV, students are allowed to shift after earning 15 units subject to compliance with the admission requirements of the new program. Upon admission, the students submit the following: application letter addressed to the Dean, true copy of grades, college/school clearance, and certificate of good moral character.

T1 (transferring within the UP System of from one CU to another) students must have completed at least 30 collegiate academic units to qualify for transfer, subject to the rules of the admitting college/school. Subjects previously cross-enrolled in another CU, if any, will not be considered for the purpose of meeting the 30-unit requirement. Upon admission, T1 students submit the following: clearance from the college /school where they came from, permit to transfer, official transcript of records, and certificate of good moral character.

Students from other universities (T2) may apply for transfer to a CU, provided that:
  1. They have earned 33 academic units in their former school
  2. They have obtained a weighted average of 2.00, 86%, or "B", or better for all the collegiate academic units earned outside of the UP System. The computation of the general weighted average (GWA) required of transfer students shall exclude vocational subjects.
  3. They will have to complete in the University no less than 50% of the units required for their program.
T2 (students from other universities) students may be admitted if the quota set by the Dean of the college/school concerned to which they seek admission has not yet been filled up. If there is a gap in enrollment, a non-enrollment certificate/affidavit must be submitted by the student.
Yes, provided that the transfer application requirements for T2 are fully satisfied. However, former UP students turned transferee-applicants may be disqualified due to these reasons:
  1. Permanent disqualification from the University. However, a student who had been permanently disqualified from UP may apply for admission to a second bachelor’s degree or to a graduate program after completing a bachelor’s degree outside the University.
  2. Dismissal for cause
  3. Withdrawn admission due to submission of fake/falsified entrance credentials/documents
  4. Found guilty of dishonesty or any form of misconduct.

Records Section

A university clearance is required for graduated students before they can officially request their diplomas or transcripts and other academic documents. It ensures that students have settled all outstanding accountabilities, such as returned borrowed materials, and complied with other university requirements in their CRSIS account. The clearance signifies that the student has fulfilled all necessary obligations. Furthermore, university clearance issued for undergraduate students means they are no longer in their program.

Graduates of the First Semester/Trimester and Midyear/Third Trimester can request their academic documents three months after the end of the term.
SEMESTER
  • The end of term for the First Semester is the day before the first day of regular registration for the second semester.
  • The end of the term for the Second Semester is the day of the commencement exercises.
  • The end of term for Midyear is the day before the first day of regular registration for the first semester.
TRIMESTER
  • The end of the term for the trimester is the day before the first day of regular registration for the next trimester.
