The UP RFID Application schedule for currently enrolled students of UP Visayas will be open on September 1-20, 2025. You may apply through this link (https://bit.ly/UPRFID). Please read through the Frequently Asked Questions (FAQ) regarding the UP RFID Application.
For the meantime, students who want to apply for UP RFID should make sure that the following requirements are ready before your proceed with your application.

Please refer to the 'Sample Picture' below on how to take your picture and renaming it before you upload. Your photo should be until your shoulder level and your information does not overlap your photo. You may also upload the same photo from your CRS account.

If, re-application (additional requirement):
- Official receipt of the payment of P130, to be paid in the Cash Office.
- Affidavit of loss (if lost or damaged) of UP ID duly notarized by an attorney.
- And/Or other necessary documents to justify your re-application.
